The enclosed information relates to the employee's Major Medical, Dental, Life, and Vision plans offered through Five Star Automotive and it's family of dealerships. As a full time employee of Five Star Automotive you will become eligible for benefits on the first day of the month following a 90 day waiting period.
Major Medical benefits are offered through Coventry Health Care of Georgia with three plan designs from which to choose. All of your Major Medical plans are PPO Select Plans.
Dental, Life, and Vision benefits are offered through The Guardian LifeInsurance Company.
Five Star Automotive has a Section 125 in place so that all premiums may be "pre-taxed" for your convenience.
