Eligibility
 


Waiting Period: An employee will become eligible on the first day of the month following a 90 day waiting period as long as the employee is considered full time.  This is true for both Coventry Major Medical and The Guardian Life, Dental, and Vision plans.

 
Open Enrollment: Open Enrollment is the period of time where an employee may make a change to their current benefits.  This will be the time to add or delete a dependent, change a plan design, delete or add to a benefit, etc.  Five Star Automotive's Open Enrollment will be in the month of October for an effective date of November 1, of each year.
 
If you are offered a benefit at your new hire effective date, but decline coverage, you must wait until Open Enrollment to enroll.  Exceptions to this rule are: birth or death of a dependent, adoption, marriage or divorce, work status change to full time or part time, or by court order.
 
 You may also apply for coverage if you have lost coverage from another policy such as a spouse's group plan.  Any changes made due to these exceptions must be made within 30 days of the "qualifying event".  These changes will require a legal document such as: marriage certificate, birth certificate, divorce papers, etc.  If you have lost coverage from another policy you will receive a "Certificate of Creditable Coverage" from that carrier and it must be turned in to the new carrier to show the ending date of coverage.
 
Dependent Children: A dependent child may stay on the Coventry Major Medical policy, whether or not they are a full time student, up to age 26.
 
For those children covered on The Guardian plans:
A dependent child must be a full time student to continue coverage at age 19.
